Glen Forman

Glen Arnold Forman Jr., 58, passed away on the morning of June 4, 2012, at Garden Manor in Brookville, Ohio. He died from complications caused by a fall in February that broke his neck and left him a quadriplegic.

Glen was born Feb. 26, 1954, in Middletown, Ohio. Although he spent most of his life in Ohio, he lived 20 years in Columbia Falls, loving the beauty of the valley and calling this his true home.

He had a heart that was golden, would give you the shirt off his back, and loved and was loved well. He will be missed by all who really knew him.

Glen was preceded in death by his father, Glen A. Forman Sr.

He is survived by his mother Elva Forman, of Columbia Falls; and three children, Shawn Forman, wife Jenny and their daughters Jasmine and Kylie, of Franklin, Ohio, Nikki Forman and her daughter Taylor, of Franklin, and Ashley Mulooenix and her children Hayden and Layla, of Bremerton, Wash.

Glen is also survived by five brothers, DeWayne Forman and wife Kim, of Rabbit Hash, Ky., Jim Forman and wife Sherry, of Walton, Ky., Ken Forman, of Trenton, Ohio, Ron Forman, of Kalispell, and Ben Forman and wife Joelle, of Columbia Falls; three sisters, Karen Waytausky and husband John, of Grove, Okla., Pam Lakes and husband Alan, of Richmond, Ky., and Debbie Forman Keller and husband Peter, of Creston; and many nieces, nephews, cousins and friends.

Until the end, Glen dreamed of returning to his Montana home where he left his heart. That dream will become realized when his son Shawn brings his ashes home in July.

A memorial service was held at First Baptist Church in Franklin on the eve of June 7, 2012.
